The easiest way I know of is go at it is from the inside of the cab.  Take
the shifter out, you probably already have, remove the boot, you probably
did to remove the shifter, and then use a reallllllyyyy long extension on
it.  I got a 24" extension from Sears for like $13 that worked great.  Then
you can use an impact on it, or in my case a 3' breaker bar.  Also a wobble
joint or u-joint adapter helps also.  Good luck.
