I don't usually, but I *have*... the main spare was on the regular carrier
that I always use, in the bed, prerunner style, and I had the extra spare
either inflated on the roof (once it was inflated) or flat (and sucked 
square) on the toolbox right behind the back window.

It's much easier to attach it to the roof than it is to suck it square and
small enough to fit in small spaces.  I have a tire evacuator that will
pull a tire down to an actual slight vacuum (after collapsing it onto the
rim) in order to get tires that flat... not quite what it was intended for,
but it does the trick.
