Grand Wagoneer Brake Parts

I am now getting to the point where I have to buy all the brake parts for
the Grand Wagoneer axles I'm swapping into my YJ.

Needless to say, I need new front calipers and rotors. The only thing usable
on the rear brakes is 1 backing plate, and even that is not real good.....
which got me to thinking, seeing as I have to replace EVERYTHING anyway, why
not try to get disk brakes in there?

Does anyone know what parts I'd need to swap disk brakes onto the rear axle
of a 1987 Grand Wagoneer Dana 44 (6 bolt)?
