I'm trying to sort out a 3VZE, 3.0 V6 that is down on power and runs a little rough. There seems to be about 180k miles on the engine. The diagnostic "check engine' system throws a code 25 (Air Fuel Ratio Lean, Malfunction). Vacuum at the PCV port on the manifold is 15" from idle up to 3000 rpm, which seems a bit low. The ignition timing is 12 degrees BTDC and the TPS and AFM ohms check out. I've taken compression readings on the passenger side three cylinders, so far. They read read 175, 168, 175 each respectively (after about three compression strokes). I bought this vehicle recently. I was told by the owner that it had a recent (in the last 20k miles?) head gasket job that was done at the automotive tech class at the local community college. The school verifies that this is the case.
I hope it's just a vacuum leak or a bad EGR -- Matt

I drove the 4runner about 30 miles today and it seemed to be running better. Maybe the gas that was in the tank was too old, it sat for about a year. Tomorrow I'll finish the compression test. I'm also gonna swap out the O2 sensor for a used one that I know is good. That low vacuum reading is something that really has me concerned. -- Matt

So I still want to hear what a good manifold vacuum reading should be for the 3VZE. I'm reading 15". I'm not absolutely sure about the accuracy of the gauge but it feels and acts like low vac. I'm thinking a healthy engine should have mid-20"s. -- Matt

My 22RE usually reads right around 20 in. vacuum at idle plus or minus a few inches. Some good vacuum gauge tips below: - http://autospeed.com.au/cms/title_Using-a-Vacuum-Gauge-for-Engine-Diagnostics/A_2393/article.html

Thanks Roger. That's a good primer on using vac gauges.
